Summary:
When MultiGet() determines that multiple query keys can be
served by examining the same data block in block cache (one Lookup()),
each PinnableSlice referring to data in that data block needs to hold
on to the block in cache so that they can be released at arbitrary
times by the API user. Historically this is accomplished with extra
calls to Ref() on the Handle from Lookup(), with each PinnableSlice
cleanup calling Release() on the Handle, but this creates extra
contention on the block cache for the extra Ref()s and Release()es,
especially because they hit the same cache shard repeatedly.
In the case of merge operands (possibly more cases?), the problem was
compounded by doing an extra Ref()+eventual Release() for each merge
operand for a key reusing a block (which could be the same key!), rather
than one Ref() per key. (Note: the non-shared case with `biter` was
already one per key.)
This change optimizes MultiGet not to rely on these extra, contentious
Ref()+Release() calls by instead, in the shared block case, wrapping
the cache Release() cleanup in a refcounted object referenced by the
PinnableSlices, such that after the last wrapped reference is released,
the cache entry is Release()ed. Relaxed atomic refcounts should be
much faster than mutex-guarded Ref() and Release(), and much less prone
to a performance cliff when MultiGet() does a lot of block sharing.
Note that I did not use std::shared_ptr, because that would require an
extra indirection object (shared_ptr itself new/delete) in order to
associate a ref increment/decrement with a Cleanable cleanup entry. (If
I assumed it was the size of two pointers, I could do some hackery to
make it work without the extra indirection, but that's too fragile.)
Some details:
* Fixed (removed) extra block cache tracing entries in cases of cache
entry reuse in MultiGet, but it's likely that in some other cases traces
are missing (XXX comment inserted)
* Moved existing implementations for cleanable.h from iterator.cc to
new cleanable.cc
* Improved API comments on Cleanable
* Added a public SharedCleanablePtr class to cleanable.h in case others
could benefit from the same pattern (potentially many Cleanables and/or
smart pointers referencing a shared Cleanable)
* Add a typedef for MultiGetContext::Mask
* Some variable renaming for clarity

* Release PinnableSlices from MultiGet as they are read rather than in
bulk (in db_test_util wrapper).
* In ASAN build, default to using a trivially small LRUCache for block_cache
so that entries are immediately erased when unreferenced. (Updated two
tests that depend on caching.) New ASAN testsuite running time seems
OK to me.
If I introduce a bug into my implementation where we skip the shared
cleanups on block reuse, ASAN detects the bug in
`db_basic_test *MultiGet*`. If I remove either of the above testing
enhancements, the bug is not detected.
Consider for follow-up work: manipulate or randomize ordering of
PinnableSlice use and release from MultiGet db_test_util wrapper. But in
typical cases, natural ordering gives pretty good functional coverage.
Performance test:
In the extreme (but possible) case of MultiGetting the same or adjacent keys
in a batch, throughput can improve by an order of magnitude.

Also note that previously, with high parallelism, having query keys
concentrated in a single block was worse than spreading them out a bit. Now
concentrated in a single block is faster than spread out, which is hopefully
consistent with natural expectation.
Random query performance: with num=1000000, over 999 x 10s runs running before & after simultaneously (each -threads=12):
Before: multireadrandom [AVG 999 runs] : 1088699 (± 7344) ops/sec; 120.4 (± 0.8 ) MB/sec
After: multireadrandom [AVG 999 runs] : 1090402 (± 7230) ops/sec; 120.6 (± 0.8 ) MB/sec
Possibly better, possibly in the noise.

The patch adds iterator support to the integrated BlobDB implementation.
Whenever a blob reference is encountered during iteration, the corresponding
blob is retrieved by calling `Version::GetBlob`, assuming the `expose_blob_index`
(formerly `allow_blob`) flag is *not* set. (Note: the flag is set by the old stacked
BlobDB implementation, which has its own blob file handling/blob retrieval logic.)
In addition, `DBIter` now uniformly returns `Status::NotSupported` with the error
message `"BlobDB does not support merge operator."` when encountering a
blob reference while performing a merge (instead of potentially returning a
message that implies the database should be opened using the stacked BlobDB's
`Open`.)
TODO: We can implement support for lazily retrieving the blob value (or in other
words, bypassing the retrieval of blob values based on key) by extending the `Iterator`
API with a new `PrepareValue` method (similarly to `InternalIterator`, which already
supports lazy values).

Cleans up some of the dependencies on test code in the Makefile while building tools:
- Moves the test::RandomString, DBBaseTest::RandomString into Random
- Moves the test::RandomHumanReadableString into Random
- Moves the DestroyDir method into file_utils
- Moves the SetupSyncPointsToMockDirectIO into sync_point.
- Moves the FaultInjection Env and FS classes under env
These changes allow all of the tools to build without dependencies on test_util, thereby simplifying the build dependencies. By moving the FaultInjection code, the dependency in db_stress on different libraries for debug vs release was eliminated.
Tested both release and debug builds via Make and CMake for both static and shared libraries.
More work remains to clean up how the tools are built and remove some unnecessary dependencies. There is also more work that should be done to get the Makefile and CMake to align in their builds -- what is in the libraries and the sizes of the executables are different.

Context: Index type `kBinarySearchWithFirstKey` added the ability for sst file iterator to sometimes report a key from index without reading the corresponding data block. This is useful when sst blocks are cut at some meaningful boundaries (e.g. one block per key prefix), and many seeks land between blocks (e.g. for each prefix, the ranges of keys in different sst files are nearly disjoint, so a typical seek needs to read a data block from only one file even if all files have the prefix). But this added a new error condition, which rocksdb code was really not equipped to deal with: `InternalIterator::value()` may fail with an IO error or Status::Incomplete, but it's just a method returning a Slice, with no way to report error instead. Before this PR, this type of error wasn't handled at all (an empty slice was returned), and kBinarySearchWithFirstKey implementation was considered a prototype.
Now that we (LogDevice) have experimented with kBinarySearchWithFirstKey for a while and confirmed that it's really useful, this PR is adding the missing error handling.
It's a pretty inconvenient situation implementation-wise. The error needs to be reported from InternalIterator when trying to access value. But there are ~700 call sites of `InternalIterator::value()`, most of which either can't hit the error condition (because the iterator is reading from memtable or from index or something) or wouldn't benefit from the deferred loading of the value (e.g. compaction iterator that reads all values anyway). Adding error handling to all these call sites would needlessly bloat the code. So instead I made the deferred value loading optional: only the call sites that may use deferred loading have to call the new method `PrepareValue()` before calling `value()`. The feature is enabled with a new bool argument `allow_unprepared_value` to a bunch of methods that create iterators (it wouldn't make sense to put it in ReadOptions because it's completely internal to iterators, with virtually no user-visible effect). Lmk if you have better ideas.
Note that the deferred value loading only happens for *internal* iterators. The user-visible iterator (DBIter) always prepares the value before returning from Seek/Next/etc. We could go further and add an API to defer that value loading too, but that's most likely not useful for LogDevice, so it doesn't seem worth the complexity for now.

For our default block cache, each additional entry has extra memory overhead. It include LRUHandle (72 bytes currently) and the cache key (two varint64, file id and offset). The usage is not negligible. For example for block_size=4k, the overhead accounts for an extra 2% memory usage for the cache. The patch charging the cache for the extra usage, reducing untracked memory usage outside block cache. The feature is enabled by default and can be disabled by passing kDontChargeCacheMetadata to the cache constructor.

Summary:
The first key is used to defer reading the data block until this file gets to the top of merging iterator's heap. For short range scans, most files never make it to the top of the heap, so this change can reduce read amplification by a lot sometimes.
Consider the following workload. There are a few data streams (we'll be calling them "logs"), each stream consisting of a sequence of blobs (we'll be calling them "records"). Each record is identified by log ID and a sequence number within the log. RocksDB key is concatenation of log ID and sequence number (big endian). Reads are mostly relatively short range scans, each within a single log. Writes are mostly sequential for each log, but writes to different logs are randomly interleaved. Compactions are disabled; instead, when we accumulate a few tens of sst files, we create a new column family and start writing to it.
So, a typical sst file consists of a few ranges of blocks, each range corresponding to one log ID (we use FlushBlockPolicy to cut blocks at log boundaries). A typical read would go like this. First, iterator Seek() reads one block from each sst file. Then a series of Next()s move through one sst file (since writes to each log are mostly sequential) until the subiterator reaches the end of this log in this sst file; then Next() switches to the next sst file and reads sequentially from that, and so on. Often a range scan will only return records from a small number of blocks in small number of sst files; in this case, the cost of initial Seek() reading one block from each file may be bigger than the cost of reading the actually useful blocks.
Neither iterate_upper_bound nor bloom filters can prevent reading one block from each file in Seek(). But this PR can: if the index contains first key from each block, we don't have to read the block until this block actually makes it to the top of merging iterator's heap, so for short range scans we won't read any blocks from most of the sst files.
This PR does the deferred block loading inside value() call. This is not ideal: there's no good way to report an IO error from inside value(). As discussed with siying offline, it would probably be better to change InternalIterator's interface to explicitly fetch deferred value and get status. I'll do it in a separate PR.

(1) the iterator represents imutable data
(2) reseek() to a larger key than the current key

because it is guaranteed that the result will fall into the same position.
This optimization will be useful for use cases where users keep seeking to keys nearby in ascending order.

`BlockBasedTableIterator` avoid reading next block on `Next()` if it detects the iterator will be out of bound, by checking against index key. The optimization was added in #2239, and by the time it only check the bound per block. It seems later change make it a per-key check, which introduce unnecessary key comparisons.
This patch come with two fixes:
Fix 1: To optimize checking for bounds, we need comparing the bounds with index key as well. However BlockBasedTableIterator doesn't know whether its index iterator is internally using user keys or internal keys. The patch fixes that by extending InternalIterator with a user_key() function that is overridden by In IndexBlockIter.
Fix 2: In #5101 we return `IsOutOfBound()=true` when block index key is out of bound. But the index key can be larger than smallest key of the next file on the level. That file can be within upper bound and should not be filtered out.

Furthermore it increases the minimum value for min_uncommitted from 0 to 1 as seq 0 is used only for last level keys that are committed in all snapshots.
The following benchmark shows +0.26% higher throughput in seekrandom benchmark.

`BlockBasedTableIterator` avoid reading next block on `Next()` if it detects the iterator will be out of bound, by checking against index key. The optimization was added in #2239, and by the time it only check the bound per block. It seems later change make it a per-key check, which introduce unnecessary key comparisons.

Summary:
The read path includes a callback function, ReadCallback, which would eventually calls IsInSnapshot to figure if a particular seq is in the reading snapshot or not. This callback is virtual, which adds the cost of multiple virtual function call to each read. The first few checks in IsInSnapshot, however, are quite trivial and take care of majority of the cases. The patch moves those to a non-virtual function in the the parent class, ReadCallback, to lower the virtual callback cost.

Summary:
This is implemented by extending ReadCallback with another function `MaxUnpreparedSequenceNumber` which returns the largest visible sequence number for the current transaction, if there is uncommitted data written to DB. Otherwise, it returns zero, indicating no uncommitted data.
There are the places where reads had to be modified.
- Get and Seek/Next was just updated to seek to max(snapshot_seq, MaxUnpreparedSequenceNumber()) instead, and iterate until a key was visible.
- Prev did not need need updates since it did not use the Seek to sequence number optimization. Assuming that locks were held when writing unprepared keys, and ValidateSnapshot runs, there should only be committed keys and unprepared keys of the current transaction, all of which are visible. Prev will simply iterate to get the last visible key.
- Reseeking to skip keys optimization was also disabled for write unprepared, since it's possible to hit the max_skip condition even while reseeking. There needs to be some way to resolve infinite looping in this case.

b555ed30a4 makes the BlockBasedTableIterator to be invalidated if the current position if over the upper bound. However, this can bring performance regression to the case of multiple Seek()s hitting the same data block but all out of upper bound.
For example, if an SST file has a data block containing following keys : {a, z}
The user sets the upper bound to be "x", and it executed following queries:
Seek("b")
Seek("c")
Seek("d")
Before the upper bound optimization, these queries always come to this same current data block of the iterator, but now inside each Seek() the data block is read from the block cache but is returned again.
To prevent this regression case, we keep the current data block iterator if it is upper bound.

Summary:
A recent change pushed down the upper bound checking to child iterators. However, this causes the logic of following sequence wrong:
Seek(key);
if (!Valid()) SeekToLast();
Because !Valid() may be caused by upper bounds, rather than the end of the iterator. In this case SeekToLast() points to totally wrong places. This can cause wrong results, infinite loops, or segfault in some cases.
This sequence is called when changing direction from forward to backward. And this by itself also implicitly happen during reseeking optimization in Prev().
Fix this bug by using SeekForPrev() rather than this sequuence, as what is already done in prefix extrator case.
